I have confirmed the following information pertaining to dry loop DSL and truck rolls.

A truck roll on the order is NOT MANDATORY for installation. A truck roll can always be offered to the customer but would generate a $199.00 charge for the truck roll if the SST rep places a truck roll on the order.

For provisioning purposes, Verizon does do an internal truck roll to 'tag the NID' - but it is not ordered through order entry nor does it cost the customer anything.
